13. Pest Control Management Agreement
14. Surat Beri Kuasa oleh Syarikat (Sdn Bhd) to PIC to run the facility (Dental Only)~Sample Click HERE

*No.1 to 9 must certified by a MO/Dr in government. No.10&11 must certified by Person In Charge PIC/Owner of the clinic.
